Appointment Reminder

  • A courtesy notice will be sent to you via email, text message, or phone call 48 hours prior to your appointment.

Day of Your Appointment

For New Patients

  • Please aim to arrive at least 30 minutes before your scheduled appointment time.
  • Ensure that you have completed the new patient paperwork before your arrival.

For All Patients

  • Remember to bring a valid photo ID and your insurance card(s).
  • Please be ready to make any necessary co-payments or cover deductibles.
  • Bring a detailed list of all medications you are currently taking.

Additional Appointment Information

We kindly ask that you honor your scheduled appointments. If you are unable to attend, please notify us promptly by telephone.

  • A minimum of 24 hours' notice is required for appointment cancellations.
  • A $35.00 fee applies for same-day appointment no-shows.
  • Missing three appointments without prior notice may result in dismissal.

Additionally, there will be a $40.00 charge for all returned checks. Patients with dishonored checks must settle future payments with cash, money orders, credit cards, or care credit.

Please refrain from bringing food or drinks into the clinic unless authorized. Childcare services are not available; please do not leave children unattended in the reception area.

Prescription Refill Process

To refill your prescriptions, please follow these steps:

  • Contact your pharmacist and request a refill.
  • Your pharmacist will send us an electronic refill request.
  • We aim to process refills within 24 hours of receiving the request. Occasionally, delays may occur, and we appreciate your understanding.
  • If additional lab tests or assessments are needed before a refill can be authorized, a member of our staff will reach out to you to discuss the next steps.

Important Information

  • Medications used in treating various conditions may have potential side effects requiring monitoring for patient safety. Our office will arrange necessary monitoring.
  • It is your responsibility to adhere to the monitoring schedule set by your doctor and attend follow-up visits to facilitate prescription refills.
  • Patients must have at least one annual visit with our providers for continued prescription refills.
  • Prescription refill requests outside business hours or on weekends will only be approved in true emergencies, with an associated service charge. Please ensure your doctor issues prescriptions during regular visits.
